Service Flow

Chronic Disease Co-Care (CDCC) Pilot Scheme is for Hong Kong Residents aged 45 or above and NO known history of diabetes mellitus (DM) or hypertension (HT). If you want to enroll the scheme, you have to register as a member of Island DHC Express, join in Electronic Health Record Sharing System (ehealth)and choose a Family Doctor (FD) for arranging blood test(s) at a designated medical laboratory. The FD make diagnosis, if the diagnosal normal result, then you are entitled to enroll free activities at Island DHC Express. If the diagnosed with DM and/or HT, or prediabetes, then entry Treatment phrase. It provide a maximum of six subsidised consultations annually and provide a maximum of four subsidized consultations annually. Then arrange necessary laboratory tests and examinations as required. Also, arrange nurse clinics and/or allied health services. The Coordination and Support from Island DHC Express is follow up and coordinate health management plan, set health goals, enhance participant’s self-health management, and help to build a healthy lifestyle. You are entitled to enroll free activities at Island DHC Express..

  • Register as a member of Island DHC Express.

FD make diagnosis

diagnosed as not having diabetes, hypertension, dyslipidemia, or other chronic diseases

diagnosed with diabetes/hypertension/dyslipidemia or prediabetes

Health Managment plan

  • Provide a maximum of 6 subsidized consultations annually to Scheme participants diagnosed with HT and /or DM;
  • Provide a maximum of 4 subsidized consultations annually to Scheme participants diagnosed with Prediabetes.
  • dyslipidemia: Up to 4 subsidized consultations in the first year, and up to 2 per year starting from the second year.

  • Arrange necessary laboratory tests and examinations as required.
  • Arrange nurse clinic and / or allied health services according to referral by the FD.

Coordination and Support from Island DHC Express

  • follow up and coordinate health management plan.
  • set health goals.
  • enhance participant’s self-health management, and help to build a healthy lifestyle.
