The Food and Health Bureau has awarded a three-year service contract for the operation of Islands DHC Express by the Neighbourhood Advice-Action Council. The Islands DHC Express has provided district-based primary medical and health services for residents or working people in outlying island districts from October 18, 2021.
In order to enhance citizens’ awareness of self-management of health and support chronic disease patients to manage chronic diseases (such as high blood pressure, diabetes, low back pain and knee arthritis, etc.), three levels of disease prevention services will be provided to the public. The first level includes health Promotion and education, disease prevention, health assessment and consultation, reduce or avoid chronic diseases, and through the second-tier health risk assessment or screening services, assist people at risk of health as early as possible through chronic disease management to avoid deterioration of their condition. The third layer of service provides patients with chronic diseases (such as hypertension, diabetes, low back pain, knee arthritis, etc.) with patient self-improvement plans and self-management of health.
