Service Target

Individuals living or working in the Islands districts includes: Lantau Island, Tung Chung, Discovery Bay, Tai O, Mui Wo, Cheung Chau, Peng Chau, etc.

Membership Eligibility

• • Holder of the Hong Kong Identity Card issued under the Registration of Persons Ordinance (Cap.177, the Laws of Hong Kong) or the certificate of exemption, except those who obtained their Hong Kong Identity Cards by virtue of a previous permission to land or remain in Hong Kong granted to him and such permission has expired or ceased to be valid; or a child who is a Hong Kong resident and under 11 years of age;
  • Those who agree to enrol the “Electronic Health Record Sharing System”; and
  • Individuals living or working in the Islands district.

Registration Procedure

Those who agree to join, please bring the following documents in person to the Islands DHC Express to complete the registration procedures:

Required Documents：

• • Hong Kong Identity Card
  • Proof of residential address with applicant’s name

The applicant shall bring along the following medical waivers for verification, if applicable, including:

• • Comprehensive Social Security Assistance
  • Higher Old Age Living Allowance (Age 75 or above)
  • Residential Care Service Voucher (Co-payment Level 0)

Member Benefits

Individuals who join as members can enjoy the following benefits:

• • Free annual health risk assessment services conducted by the DHC Express’s medical staff
  • Free enrolment in various groups, talk, activities
  • Priority registration for various promotional activities
  • Members in need can join the government-subsidized chronic Disease Management Programmes upon referral by the Network Medical Practitioners of the DHC Express.
